Forms Completed Before ResearchDescription
Non-Human Vertebrate Animal Exceptions All information will be entered into the online entry system and an upload of the IACUC approval will be requested. No additional paper form is necessary.
Institutional Review Board (IRB) Approval Form for Research with Human Subjects All projects involving humans, including surveys, must obtain IRB approval.
Informed Consent Statement for Research with Human Subjects All human subjects projects that are determined to contain more than minimal risk by an IRB must have Informed Consent. Please review the Intel STS  Rules and Instructions for information on conducting a risk assessment.
